POLST UPDATE

AB 3000 (Wolk) was signed into law by the Governor on August 4. This new law ensures that when a patient has a completed Physician Orders for Life-Sustaining Treatment (POLST) form, the form must be honored by all health care providers.  A POLST form addresses a range of treatment options and enables a patient to clearly express their treatment preferences regarding life-sustaining treatments such as resuscitation, nutrition, and pain management.

Click here, to read the California Chronicle's April 23, 2008 article on AB 3000.

If you have any questions about POLST or AB 3000, please contact Judy Citko at (916) 552-7573 or jcitko@finalchoices.org.

California Department of Public Health Letter to Health Care Facilities

The Letter applies to hospitals, nursing homes, home health agencies, hospices, and all other health facilities licensed by the Department. The Letter serves as notice and a reminder to health facilities regarding the provisions of AB 3000 (Wolk).
